本人知悉及同意青苗基金在活動進行期間對本人進行拍攝或錄影，本人同意貴會及相關支持機構有權使用該等影像及影片，連同本人在活動中自行拍攝的影像和影片，以及有關活動之分享，作紀錄及宣傳之用。I give my consent to YAF to take my photograph or film me during the program, and that the pictures or videos taken will be used for documentation or promotion purposes by YAF and any supporting organisations. I also give my consent to allow YAF and any supporting organisations to use any/all images and/or videos and/or testimonials taken of or provided by me about the program for the above purposes.
